At Justa Farm Dental Esthetics, we value your time and strive to process all the necessary paperwork as quickly and efficiently as possible. For your convenience and to maximize your time, we ask that you complete the following paperwork at home and bring the completed forms with you on your first visit. If you have any problems printing these forms, please let us know immediately. We are here to help.
Payment Methods
Because everyone’s finances are different, we provide numerous payment and deferred payment options. Our patients can choose from 90-day no-interest plans to 60-month payment plans, all offered through Chase Health Advance and Care Credit. We also accept Visa, MasterCard, Discover, and American Express.
Insurance
Justa Farm Dental Esthetics accepts almost all major dental insurance plans. We help our patients maximize benefits. However, due to the high quality of care for which we strive, we are unable to accept HMO plans. If you have any questions about financing your dental procedures, please feel free to contact us.
